PCOS Fertility GPS™ Privacy Policy
PCOS Fertility GPS™ is an educational wellness tool, not medical care. It does not diagnose or treat any condition and is not a substitute for individualized medical care from your own licensed clinician (for example, your physician, OB-GYN, reproductive endocrinologist, or fertility clinic). Always consult your own provider about your health.
1. Who operates this application
PCOS Fertility GPS™ is operated by Love Served Warm. If you have questions about this policy or your information, contact us at Privacy@loveservedwarm.com.
2. Scope of this notice
This notice describes how the PCOS Fertility GPS™ web application handles information. It covers the assessment, the results experience, the email sign-up, and the optional saved-map login. It does not govern third-party services you may be handed off to (such as the booking platform), which have their own policies.
3. Assessment information we process
When you take the assessment, your answers are used to calculate your Fertility Route Score™ and generate your Fertility Map. Your raw questionnaire answers are not stored. We store only the derived scores and the finished, rendered result needed to show your Fertility Map. Two optional personalization questions and the pregnancy screening answer are used in the moment and are not stored.
4. Information stored in our application database
We store: your derived assessment scores and rendered result; your first name and email address (once you provide them to unlock or receive your results); your marketing-consent history and current marketing preference; and internal, non-identifying record identifiers.
5. Information we intentionally do not store
We do not store your raw questionnaire answers, the optional personalization answers, or the pregnancy screening answer. Your private assessment answers and scores are kept within this application and are never sent to our email platform.
6. Email sign-up to take the GPS
To start the GPS, we ask for your first name and email. We use them to send your results, to let you get back to your map, and to send fertility education and occasional updates from Tianna Trinidad and Love Served Warm, as the notice next to the email box explains. Starting the GPS after entering your email is how you sign up.
7. Unsubscribing
You can unsubscribe from Love Served Warm emails at any time using the link in any email. You can still use the GPS and your Fertility Map after unsubscribing.
8. Sign-up records and current email preference
When you sign up or change your email preference, we keep an append-only record of that decision — what you agreed to, when, and under which notice version — as evidence during normal operation. Separately, we keep your current marketing preference (allowed or not) so we always honor your latest choice. A later change adds a new record and does not erase the earlier one.
9. Transfer to Kajabi (via Zapier)
When you sign up, we send a minimal record to our email platform, Kajabi, through a Zapier webhook: your first name, email, a source label, and your sign-up details (when and under which notice). We never send your assessment answers, scores, results, cycle, pregnancy, medication, lab, symptom, or any other health information to Zapier or Kajabi.
10. Hosting (Railway)
The application and its database are hosted on Railway, which processes and stores the information described above on our behalf.
11. Optional saved Fertility Map (Supabase)
Saving your Fertility Map is optional. If you use it, Supabase provides an email-based login and stores one saved, display-ready copy of your Fertility Map for your account. This saved copy does not include your raw answers, and it is separate from marketing. This login is not required to use the assessment, view results, or book.
12. Booking hand-off
If you choose to book the paid assessment, the booking button takes you to the Love Served Warm assessment page (loveservedwarm.com/assessment). Booking from that page is handled by Practice Better, and any information you enter there is handled by Practice Better under its own terms and privacy policy, not this one.
13. First-party application events
We use simple, first-party product events to understand how the tool is used. The event endpoint validates events and rejects any that would carry personal or health fields; it does not store your answers, results, name, email, or pregnancy status.
14. No third-party advertising or session tracking
This application does not include third-party advertising pixels, behavioral retargeting, session-replay, or heat-mapping tools.
15. Saved map, sign-out, and other deletion
These are four different actions:
- Delete My Saved Fertility Map removes only your saved Fertility Map. It does not sign you out, delete your login identity, or delete your contact, email, or marketing records.
- Sign out ends your current login session in the browser. It does not delete your saved map, your login identity, or any of your information.
- Deleting your contact or account information — use our privacy request form. After you verify your email, our team deletes the operational records we control and tracks any remaining steps.
- Deleting your login (Supabase) identity — also request this through the privacy request form. This is completed by our team as an administrative step; it is not automatic and is not reported complete until it is actually done.
You may also contact Privacy@loveservedwarm.com. We do not offer a fully automated, instant deletion of every record across every system.
16. Unsubscribing and a current limitation
You can unsubscribe from marketing emails at any time using the link in those emails, which is handled by our email platform. At this time, an unsubscribe made in the email platform is not automatically reflected back into this application; if you want your in-app marketing preference updated as well, contact Privacy@loveservedwarm.com.
17. Retention and vendor backups
We keep information for as long as needed to provide the service and to maintain consent records, then reduce or remove it. Our service providers (for example, Railway, Kajabi, Zapier, and Supabase) may keep their own backups on schedules we do not control, so removal is not instantaneous everywhere. Some assessment records are anonymous and may not be linkable to your email.
18. Your privacy choices
You can ask us to access, correct, or delete your information, or to withdraw marketing permission, using our privacy request form. We first email you a one-time code to verify you own the email address. After you verify: withdrawing marketing permission is processed automatically; other requests (access, correction, deletion) are handled by our team. Not everything is instant or fully automated — some steps, including any action needed at our email platform or other vendors, are completed manually, and a request is not marked complete until those steps are done.
